Please note: Referral does not guarantee acceptance to our service. Each case is reviewed in detail to ensure the most appropriate care. Our team will be in contact within 48 hours with an acknowledgement of receiving your referral and to advise you of the next steps.

to submit an online ‘Client Intake Form’

Click Here

to download the form and complete in MS Word and send us an email

Private paying clients i.e. private health insurance, EPC, full fee paying

Click Here

Step 2

You will be advised within 48hours if the referral is appropriate for our service or requested to provide more information to assess suitability.

Step 3

Our Client Pathways Coordinator will then make contact to schedule an initial assessment with a clinician.
